About the Role
We are seeking an experienced Nurse Manager II – Utilization Management (UM) / Discharge Planning to lead care coordination and patient flow initiatives in a fast-paced acute care hospital environment. This role is ideal for a strong clinical leader with expertise in case management, utilization review, and discharge planning.
What You’ll Do
- Lead daily operations of UM and discharge planning functions
- Oversee patient care coordination and ensure smooth transitions of care
- Manage and develop case management and nursing teams
- Ensure compliance with regulatory standards (Joint Commission, Nursing Practice Act)
- Monitor KPIs related to patient flow, length of stay, and quality outcomes
- Collaborate with physicians, care teams, and external providers
- Drive process improvements to enhance efficiency and patient outcomes
- Manage staffing, resource utilization, and departmental performance
- Support strategic planning and operational initiatives
What You Bring
- Bachelor’s Degree in Nursing (BSN) required
- Active California RN License
- BLS Certification
- 5+ years of patient care experience
- 4+ years of leadership experience
- Strong recent experience in Utilization Review (UR), Case Management, or Discharge Planning in an acute care hospital
